COMMUNITY HEALTH PROMOTION
8 Week-Weight Loss Program

Community Health Promoters of the Somali and Latino communities have partnered with the Matt Griffin-YMCA and the Tukwila Community Center to run an 8-week weight loss program that includes weekly physical activity and personalized support over the phone. Our goal is to close the gap between clinical care and a healthier lifestyle by enrolling Latino and Somali HealthPoint patients that have been referred by their provider. We offer classes that suit the needs of each community and address barriers such as language, childcare and motivation.

CONNECTION DESK

In May 2013, G2L launched the Connection Desk – staffed by University of Washington and Seattle University students – as a community resource to help navigate health and social service systems

HealthPoint physicians refer medical patients in need of social services to the G2L Connection Desk to identify opportunities for food, housing, employment, language skills training and other available social services to help address the underlying social issues that often drive poor health outcomes

The Connection Desk also refers clients to G2L’s Community Health Promoters and the range of services that they offer

MOBILE HEALTH PROGRAM
Remote Case Managing for Diabetes, Weight Loss and Smoking Cessation

G2L's Mobile Health Program uses innovative technologies to approach chronic disease management in the SeaTac and Tukwila region of South King County. With the success and findings from the program's diabetic pilot study, G2L's Mobile Health program has now expanded its services to offer remote case management for weight loss, smoking cessation and diabetes. We offer customized, one-on-one support to meet each of our participant's needs.

FOOD INNOVATION NETWORK

The SeaTac-Tukwila Food Innovation Network is comprised of public and private partners sharing a common agenda to address health and income disparities through facilitating growth in the local food sector by creating jobs and entrepreneurship opportunities in the SeaTac and Tukwila communities.

HEALTH SCREENINGS

In partnership with Novo Nordisk, Bartell Drugs, and Abbott Labs, G2L hosted three community health screening events where more than 400 people received approximately 1,600 different screenings

Approximately 25 percent of the individuals screened were referred to a primary care provider for immediate attention
-
Many screening participants have since reported lowering their cholesterol and blood glucose levels significantly. Participants also commented that they were previously unaware of their medical conditions and were now empowered to improve their diet and exercise
